Course Content

Problem solving ability

The ASO sharpens its own problem-solving skills during the various seminars, scientific meetings and daily clinical work.
Teaching methods:
- Daily work in the operating room, emergencies, Intensive Care and remote locations
- Interactive seminars: 1h per week (own time, also 1h each time also supervised by master students)
- Evening seminar: 1x per 2 months 2 hours (once a year 25 hours for your own session)
- Morbidity staff meeting: 4 x per year (preparation 1 h; presentation each time 2-3 ASO and member of staff of cases)
Skills

The ASO is given the opportunity to practice a wide range of fields within the specialism during its training. During these internships, ASO acquires specific knowledge and skills and applies them in practice.

Internships during the training are primarily these specialties in which core competencies must be obtained:
- anesthesia for obstetrics
- anesthesia for airway management and surgery
- anesthesia for chest and / or cardiovascular surgery
- neuroesthesiology
- anesthesia for orthopedics and traumatology
- pediatric anesthesia
- anesthesia for remote examinations: ambulatory anesthesiology

In addition, there are 10 domains of general competence and skills:
- Disease management, preoperative risk assessment, preparation
- Intra-operative care and anesthesia management
- Post-operative management and acute pain treatment
- Emergency medicine
- Multidisciplinary intensive medicine and perioperative care

1. General medical knowledge and skills

1.b. Knowledge:
- preoperative consultation: targeted anamnesis and clinical examination, including assessment of the operative risk
- targeted use and interpretation of lab, imaging
- basic knowledge of ultrasound and echocardiography
- dealing with differential diagnostic problems, assessment, interpretation
- motivated decision-making regarding anesthesiological problems preoperatively, intraoperatively and postoperatively
- immediate postoperative treatment, including postoperative acute pain treatment
- basic knowledge of hemostasis, clotting and anticoagulants
- medico-legal aspects
- interdisciplinary communication before and after the procedure

1.c. Skills:
- the preoperative information meeting

2.c. Skills:
- placement of chest drain
- intubation and manual ventilation
- surgical protection of the respiratory system

3. Intensive care

3.b. Knowledge:
- principles of invasive and non-invasive patient monitoring
- principles of hemodynamics and hemodynamic support, both pharmacological and mechanical
- Ventilation and weaning techniques of ventilator
- treatment of hypovolemic, cardiogenic, distributive and obstructive shock conditions
- basic knowledge of organ replacement techniques
- basic knowledge of infectiology and treatment of serious and life-threatening infections, sepsis and septic shock
- support and treatment of neurotrauma and postoperative complicated intracranial pathology
- criteria of brain death
- principles of organ donation

3.c. Skills
- accommodating unstable hemodynamic or / and respiratory compromised or deteriorating patient
- urgent intubation and ventilation
- introduction of catheters for monitoring, infusion management, pharmacological or mechanical hemodynamic support

4. Anesthesia for thorax and neck surgery

- clinic and diagnosis of thoracic and neck disorders
access routes to thorax, pericardium, mediastinum and neck (including closure and
  complications)
- endocrine evaluation of thyroid and parathyroid gland
- principles of minimally invasive chest surgery
- assessment of lung function
- exhibition techniques
- principles of oncological chest and neck surgery

4.c. Skills:
- in connection with single long ventilation (placing a double lumen tube and assessment of correct placement, bronchial blocker, EZ blocker, etc.).
- perioperative pain relief after chest surgery

5. Cardiac surgery

5.b. Knowledge:
- clinic, diagnostics and follow-up of cardiac syndromes, ischemic heart disease and valve disease
- (patho) physiology of hemostasis, coagulation and pharmacology of anticoagulants; anticoagulation monitoring
- (patho) physiology of arrhythmias, antiarrhythmics and pacemaker therapy, antihypertensive agents,
  valve pathology, including endocarditis
- basic knowledge of the use of extracorporeal circulation during cardiac surgery and on Intensive Care

6. Child surgery
