Multi-disciplinary discussion (MDO): (at least 1x/week, 2h) with evolution and active participation in leading the discussions and develop a multi-disciplinary care pathway that is supported by the whole team (supervised by staff members/specialists in Geriatrics).
Communication with all members of the multi-disciplinary (MD) team (e.g. nurses, occupational therapists, physiotherapists, speech therapists, psychologists, dietitians, clinical pharmacists...) both content- and patient-targeted, also organizational and supportive (daily). The ASO is expected to gradually require less supervision by staff members and take the initiative if necessary.
Communication with patients (daily) and families (at least 1x/week, 30mins) regarding the medical situation and diagnosis and the further policy. Special attention goes to the patient’s individual goals and priorities (also towards the end of life), communication of (if possible) evidence-based therapy proposals and the intensity of the provided care (futile examinations and interventions vs. useful fine-tuning and efficient treatments, even at old age).
Bedside teaching for staff member specialists in Geriatrics to recognize non-verbal communication (e.g. pain) in demented elderly people and adequately communicate with patients with memory disorders and/or disorientation and/or insanity.
Within the activities of the Care Program for Geriatrics (ZPG), the ASO participates in the advices and internal liaison activity, in which patients with a geriatric profile in other departments are systematically traced and approached (not only on a colleague’s request). Approaching both the patient and the responsible colleague, requires a certain amount of respect and persuasiveness, to explain the geriatric aspects and implement necessary interventions.
